Learn more →Who Qualifies
PACE is designed for seniors who need nursing-home-level care but prefer to live at home or in the community.
Age 55 or older — PACE is specifically designed for older adults who need comprehensive long-term care.
Nursing-facility-level care need — Must meet the state's clinical requirements for nursing home care.
Live in a PACE service area — Must reside within the geographic area served by a PACE organization.
Able to live safely in community — Must be able to live safely at home with PACE support services in place.
Medicare or Medicaid eligible — Must be enrolled in or eligible for Medicare and/or Medicaid.

Our interdisciplinary team evaluates your medical, functional, and social needs — at home or at the PACE center.
Your team creates a tailored care plan based on your unique goals, needs, and preferences.
